Chargers Volleyball Club Tryout Waiver & Release Form

I give __________________________________ (participant name), permission to participate in Chargers Volleyball Club activities.

By signing this form, I acknowledge that I have read, understand, and agree to the following:

1. I understand that participation in volleyball activities as an athlete or coach can be dangerous and may result in minor or major injuries, including catastrophic injury, paralysis, or death.

2. I authorize any qualified staff member to secure medical treatment or emergency transportation if necessary in the event of injury.

3. I understand and agree to follow all facility usage expectations and rules pertaining to volleyball courts and training areas. Failure to comply may result in disciplinary action or removal from participation.

4. I certify that the participant has active medical insurance coverage through the provider listed below.

5. I voluntarily consent to participation in Chargers Volleyball Club activities and understand the risks associated with volleyball and athletic training activities. Direct adult supervision may not be present at all times.

6. I, my executors, guardians, or representatives waive and release all rights and claims for damages against Chargers Volleyball Club, its coaches, staff, volunteers, and representatives for any injury or damages resulting from participation in club activities.

7. I confirm that I am at least eighteen (18) years old or am the parent/legal guardian of the participant listed above.

* players ARE REQUIRED  to have an active Badger Region/USAV membership to participate in tryouts at Chargers Volleyball Club. If you do not have a current full season membership valid through 8/31/2026. You will need to purchase a tryout membership, which will become available July 1st
